Who can contact Grace?

Adults seeking support, family members, guardians, support coordinators, and other planning partners may contact Grace to begin a conversation.

Do I need to know the exact service before I call?

No. Describe the person’s goals, routines, preferences, current supports, and the challenge you are trying to solve. Grace can help clarify which page or conversation may be relevant.

Does contacting Grace guarantee acceptance or placement?

No. A conversation does not guarantee eligibility, authorization, funding, staffing, placement, or service availability.

Eligibility and funding

Does Grace determine NJ DDD eligibility?

No. The New Jersey Division of Developmental Disabilities makes DDD eligibility decisions. Grace can explain its own services and direct visitors to official state guidance.

Can Grace promise that Medicaid or NJ FamilyCare will pay?

No. Public agencies and approved plans determine eligibility, covered services, and authorization. Verify current requirements with the responsible agency and your planning team.

Does the Housing Supports program pay rent?

No. Housing Supports may help an eligible person find, move into, and maintain housing, but the program does not pay rent.

Are all services available in every county?

Not necessarily. Availability depends on the requested service, location, current capacity, staffing, funding, authorization, and individual fit.

Which New Jersey counties can contact Grace?

Grace’s approved county list includes Mercer, Burlington, Middlesex, Monmouth, Ocean, Hunterdon, Somerset, Morris, Union, Essex, Camden, Gloucester, and Hudson counties.

What happens next

What should I prepare for the first conversation?

Bring the person’s goals, preferences, current living situation, existing services, county, and any information you have about DDD, Medicaid, NJ FamilyCare, or an approved plan. It is okay if some details are not yet known.

Can I send a referral online?

Yes. Use Send a Referral for Support for a general question or Schedule an Appointment when you are ready to share more detail through the Participant Referral Form.
